To connect/link a form to a Gallery, you’ll go to the set up of the intended Gallery. First, you will want to delete the default form by clicking the black X.

You’ll then click (+ Add a Source Or Entry Form). Upon clicking, a dropdown will appear. Click on it and select Use Contest Form.

Next, you'll see a list of all the contest forms you can pull in to the Gallery.

Select the desired Contest Form form you would like to add. Upon selection of the Contest, its form will be connected to your Gallery.
